NOTARY PUBLIC - Instructions For Filling Out an Online Form

1. After activating the form using Adobe Acrobat Reader, select the hand tool in the toolbar, if it is not already selected.

2. Move the "hand" cursor to an underlined field or to a checkbox. You'll know you're in an updateable field when the hand cursor changes to an I-beam or to an arrow.

3. When the hand changes to an I-beam, click your left mouse button, then key in the field. For checkboxes, when the hand changes to an arrow, click the left mouse button to put a checkmark in the appropriate box.

4. After entering text in a field, do one or the following to proceed:

   • Press Tab to accept the field change and go to the next field.

   • Press Shift+Tab to accept the field change and go to the previous field.

   • Press Return or Enter to accept the field change and deselect the current field.

   • NOTE: In a multiline text field (indicated by a box), Return goes to the next line; it does not accept the field change or deselect the current field.

   • Press the Escape key to reject the field change and deselect the current field.

   • NOTE: Be sure to key Return or Enter after filling in the last field before printing. If you do not accept the field change and deselect the last field, the information that you keyed into it will not print.

5. Save the form to your local drive, print it out, and sign it.
